Privacy is important to MinibarRx, LLC and the entities with which it contracts to perform certain non-medical services (collectively, “MinibarRx”); MinibarRx strives to be a responsible participant in the health care industry. MinibarRx’s corporate policy is to protect the privacy and confidentiality of its customers and those individuals whose health care information may be identified within the services supplied by MinibarRx to its customers. This Privacy Policy describes what information MinibarRx collects and how that information is protected and used. MinibarRx operates under the following set of strict privacy principles:
Except as disclosed in this Privacy Policy, information supplied by a physician or patient that could be used to identify or contact a patient, such as name, e-mail address or other similar information (“Personal Information”), will not be disclosed to anyone unless the patient indicates that MinibarRx may do so or unless permitted under a Business Associate Agreement (as hereinafter defined) between MinibarRx and the physician. Personal Information will be maintained and protected by MinibarRx as “Individually Identifiable Health Information” in accordance with the requirements and definitions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996 (Public Law 104-191) and any implementing regulations. MinibarRx will also observe all other state and federal laws relating to transmission, storage, and access to medical records and other health care data.

MinibarRx may share statistical deidentified information about its customers as a group, such as usage habits, demographics and similar information that cannot be used to identify or contact patients (“Aggregate Information”) with various third parties.

Disclosure of Personal Information

Any Personal Information provided to entities affiliated with MinibarRx will be treated in accordance with the terms of this Privacy Policy. Business Associates
If you are a health care provider or other individual or entity with an existing service or other agreement with MinibarRx (a “Business Associate Agreement”) under which MinibarRx or its agents may have access to patient information, you need to be sure that your Business Associate Agreement contains certain provisions to ensure compliance with HIPAA and this Privacy Policy. Upon request, MinibarRx provides a standard form of amendment to Business Associate Agreements for this purpose.

MinibarRx combines industry-standard technical safeguards with employee policies to help prevent the loss, misuse or alteration of Personal Information under its control. Only authorized MinibarRx employees, agents or independent contractors are permitted to access your Personal Information, and all such employees, agents and independent contractors must agree in writing to abide by the MinibarRx Privacy Policy. Those who violate the Privacy Policy are subject to disciplinary action, up to and including termination. MinibarRx employs strict security measures to safeguard online transactions; Personal Information is stored in secured systems and is always encrypted when sent via Internet channels.
